As our mission is to provide integrated, open-access healthcare for all, we offer a Sliding Fee Discount Program to those who qualify. What is a sliding fee discount?
The Sliding Fee Discount enables NEW Health to offer a schedule of fees or payments for qualifying services based on the patient's ability to pay. This sliding fee discount can apply to all household members and can last up to one year with the proper documentation.
How is a reduction in fee determined?
The Sliding Fee Discount is determined based on income level and the number of members in the household. Using this information, our staff computes the amount of the sliding fee discount based on annually published federal poverty guidelines. Depending on the level of sliding fee qualified for, a percentage of the visit service charges will be discounted.
How can I qualify for a sliding fee discount?
To qualify for a sliding fee discount the patients income must be at or below 200% of the federal poverty guidelines. Patients need to provide documentation of their income and the number of members in their household.
What type of documentation do I need to provide?
Copies of wage statements, unemployment/pay stubs, etc., may be used for income verification. A Self-Declaration form may be used if other forms of verification are not available. A copy of a current driver’s license or photo ID will also be requested. This information is only used for determining your eligibility for a sliding fee discount and is held in strict confidence.
