NEW Health is designated as a Patient-Centered Medical Home (PCMH), a team-based model of care led by your primary care provider. Your team provides continuous, coordinated, and evidence-based care throughout to maximize health outcomes. You are at the center of your care team. Your team can create a personalized and coordinated care plan and stay connected via our patient portal messaging and after-hours phone access.

How to optimize your PCMH experience

  • Be an active partner in your care: ask questions, communicate your health goals and concerns to your care team members, and inquire about self-management options.
  • Provide a thorough health history, including records from prior providers and specialists.
  • Collaborate in the development of your care plan and goals and follow your plan of care.
  • Keep your scheduled appointments.

More about the PCMH model

In the PCMH care delivery model, patient treatment is coordinated through primary care providers to ensure patients receive the necessary care when and where they need it in a manner they can understand. There are six overarching PCMH themes

Team-Based Care and Practice Organization

Helps structure a practice’s leadership, care team responsibilities, and how the practice partners with patients, families, and caregivers.

Knowing and Managing Your Patients

Sets standards for data collection, medication reconciliation, evidence-based clinical decision support, and other activities.

Patient-Centered Access and Continuity

Guides practices to provide patients with convenient access to clinical advice and ensure continuity of care.

Care Management and Support

Helps providers set up care management protocols to identify patients who need more closely managed care.

Care Coordination and Care Transitions

Ensures that primary and specialty care providers effectively share information and manage patient referrals to minimize cost, confusion, and inappropriate care.

Performance Measurement and Quality Improvement

Improvement helps practices develop ways to measure performance, set goals and develop activities to improve performance.
