How to optimize your PCMH experience

  • Be an active partner in your care: ask questions, communicate your health goals and concerns to your care team members, and inquire about self-management options.
  • Provide a thorough health history, including records from prior providers and specialists.
  • Collaborate in the development of your care plan and goals and follow your plan of care.
  • Keep your scheduled appointments.
