Beginning Thursday, March 31, 2022, NEW Health medical clinics will begin offering Pre-Appointment Forms to patients before their appointments.

After making an appointment at a NEW Health clinic, you will receive a text message and/or email from NEW Health 4 days before your appointment. This will guide you via a secure link to digital Pre-Appointment Forms that you can complete on your smartphone, tablet, or computer. These Pre-Appointment Forms will gather or update your important information, such as demographics, health screeners, health history, insurance information, and consent forms. Completing these digital forms will save you time at check-in when arriving in the clinic for your appointment, and will help NEW Health maintain accurate health records for you.

This message is for informational purposes only; you don’t need to take any action to begin using NEW Health’s digital Pre-Appointment forms. If you are unable to complete these forms digitally, you will still be able to fill them out on paper when you arrive at the clinic for your appointment.

How does the process work?

You will receive a text message and email four days before your appointment that will contain a secure link. The link will prompt you to complete the pre-appointment forms. There is no login, password, or download needed to complete these digital forms. The information you input is saved automatically, so you won’t lose your work if you need to stop in the middle of filling them out. And the digital forms are connected to our HIPAA-compliant patient record platform, so the information automatically gets uploaded to your medical record.

If you do not complete the pre-appointment forms you will receive a reminder text and email two days before your appointment to complete the forms.

Why did NEW Health implement this process?

Implementing pre-appointment forms will help NEW Health maintain accurate and up-to-date medical records while reducing check-in time for patients once they are in our clinics. These digital forms also reduce manual input errors and will create a smoother check-in process for both our patients and our staff.

What devices can I use to fill out the pre-appointment forms? What if I don’t have any of those options?

You can use a smartphone, tablet, computer, or any other device that connects to the Internet. If you don’t have access to any of these device types, or do not have access to the Internet, you will still have the option to fill out paper forms in the clinic.
